每 [měi: every] like the other demonstratives we learned (這 那 and 哪) generally requires the use of a measure word between 每 and a noun.
Some nouns like 天 and 課 behave like measure words, so do not require additional measure words. For some few nouns like 人 and 班 the MW is optional.
每 + [MW] + Noun
Generally 都 is added to the predicate of a 每 sentence to highlight the the all-inclusive meaning.
每 + MW + Noun + 都 + Verb phrase
Since 都 must come before the verb in a sentence, when 每 is used in the Object position 都 is not used.
Nor is 都 used in questions using 幾 or 多少. It is also not used in the response to these questions.
